was present at a recent adobe presentation on adobe's accessible pdf format,
one of the more problematic issues that arose was the small problem of adobe
acrobat itself not being and accessible package, and the default setting is
(or was not then) to output accessible files.  You had to make a conscious
decision to add the additional info. 

It was therefore likely that you would end up with two forms of pdf 5, the
accessible ones were editors had made the effort to create them, and the non
accessible ones where editors had not or were unaware of the potential.

In fairness i have not investigated this further since, the so they may have
addressed these.
