favicon ??

I have been following the discussion on IE5 and favicon.ico

I have set up an icon file using Microangelo Studio (favicon.ico) and
placed it in the root directory of the web site. But when the index page
is added to the favourites only the default windows icon (not the IE
icon) shows in the favourites list.

I am obviously doing something wrong. Does the icon file have to be a
certain size (I have tried it at both 16x16 and 32x32) ?
Does the colour set have to be a certain depth (currently at 256
colours) ?
Does specific software have to be used to set up the icon (anyone know
of anything other than Microangelo?).
When the 'root' directory is specified does it have to be the very
bottom - I have a number of different subjects from the main root (e.g.
www/consult-eco.ndirect.co.uk/hbc/      and
www/consult-eco.ndirect.co.uk/wncf/  ) and I would like to put
favicon.ico files in each of these for differing users. Can this be
done?
Have I got the name of the icon file wrong ?? I have tried both
favicon.ico  and also  favico.ico  ??
