changing hosts

We are moving one of our sites from a hosting company to an inhouse server
and I am in charge of making the changes with Network Solutions.  I've never
dealt with them before but see they have several forms for making various
changes.  I want to make sure I change the right thing so can someone please
tell me what form I need to fill out?
