Above the head?  You mean IN the HEAD section?  That's the only place I ever
put it.  I have a simple example with plenty of comments in it at
http://www.betterwebdesign.com.au/example/  Perhaps this is of some use?

Or there is an extensive, more involved one on my main site - see first URL
in signature.  It does a lot more than just mouseovers so it might not make
sense.  But once again, the functionality is in the HEAD section...
