Your ISP does not accept index.htm as a default page. Contact them to find
out what they have set for a default. It could be index.html, or default.htm
or default.html which are the most common.

Have you checked with them to see if they have a counter available? If not
and they don't give you any CGI access either there are many counter
services available at no cost.
