Database Construction

I would like to construct a form that collects data which dumps into a
database, Access if possible.

I presently use FP2000 for page/site construction.  I can also use Allaire's
Homesite, but since I am pretty much an html dummy, I tend to use FP.

Can someone give me some guidance in how to best do this?  Where to look for
resources?  Some designers that might do this on the fly relatively "cheap?"

When I did some research years back, I had a web designer do this type of
task.  He constructed the forms, and also the database.  He actually gave me
a link to a URL where I would go and download the database onto my desktop,
and I crunched the numbers from there.

Any help would be appreciated.
