Recommend Site Script

I apologize if I am posting a question that has already been asked, but as 
you know the Archives are currently not available. I am looking for an 
easy-to-install and free CGI script that allows a visitor (let's call him 
John) to my web site to recommend the site to ***multiple*** friends. In 
other words, my requirements are:

1. A one-page HTML form that contains multiple fields to enter multiple 
names and email addresses.

2. A hidden field to send a text message to these friends; I don't want 
John to be able to edit the text message.

3. Send an email message to "John," thanking him for referring his friends.

The reason I want John to get an email is I am going to give John a 
discount on our services for referring his friends. The email will include 
a special code for verification purposes.

I hope I am making sense. I have looked high and low on the web for such a 
script; I can find plenty of scripts that fill one of these requirements, 
but not all 3. I have looked at Bignosebird.com (Birdcast), Cliff's 
scripts, CGI World, CGI City, Find A Script, ScriptSearch, Get Perl (Easy 
Refer), and many, many more.

Please don't spend any time looking for something to fit my requirements. 
I'm just hoping you know of something off the top of your head that you can 
recommend.
