Kathie,
Move your opening FORM tag before your opening TABLE tag. However, you also
have other problems. At the end of yor document you have an extra set of
</td>
</tr>
</table>
tags.
You also need to move your closing DIV, and you have a TABLE tag that is not
closed. One TABLE opens on line 14, the other on line 36.
