[Photoshop] Non-angular corners

Hello oldies, newbies, in between and the like-
Have a question for some of the more experienced folks out there.
I'm kinda embarrassed that I've made it this far without figuring out how
to do this:
How do I make a square box, with a thickness of about 4 or 5 pixels, but
keep the edges rounded, curved?
I have tried making a rectangular selection, then holding shift and adding
circular selections to the edges, but this is way too inconsistent, as it
never works like I want it too. I am just about to get stewed over this one.
I have worked with this problem for months, hesitant to ask for help; call
it pride, whatever-- I just like to try to get it on my own, but heck, I'm
at a loss. Am I missing a simple key-press?
HELP! :)

Notice the retangular box with smooth, rounded, symmetrical corners on the
second layer of the image. It's so perfect, I think it's laughing at me now.
