I have been doing POS (Point of Sale; wine list, table tent, menu) for local
service establishments for quite some time, generally through illustration
with a few computerized effects. In the last two years I have turned my
interests toward internet related advertising, and that has definitely
involved learning and creating a whole new style and format of graphic.
My files have expanded so far outside my hard drive that I just bought a
second zip drive. I have a PC in my home office and a laptop as my mobile
office. I have started storing all of my files on zip disks. I have enough
room to keep about 6+ projects on each disk, along with lots and lots of
random images, samples, fills, color schemes, relative text and what not that
pertains to each of these projects. 
As far as the files go, I am not too different than you in the creation and
storage of a file into a folder. I start with an idea or a subject, and as I
work on it I save it in stages, and in different formats, into a temp folder
under that project folder.  As I get closer to a finished product, I go back
and clean out images I know I will never use, and renaming and relocating
files more appropriately. 
As far as locating or recognizing files or images I've created earlier, I use
PSP's thumbnails (browse folder) to refresh my memory. I can view everything
in the folder and pick out the things I need to work on (or delete).
