Looking for a Script

Does anyone have or know of a script (that can be gotten from a site, for 
instance) -- JavaScript, VB, ASP or otherwise that will do the following (or 
what this action would be called):

I have a Volunteer Form I want to put on my multimedia group's Web site. This 
form has an area for the person to fill in his/her name and contact info. 
Under this area are categories of tasks they can volunteer for.

I want the form to be sent to various people, depending what the person is 
volunteering for.

IE: A person fills out name and contact info. He or she volunteers to write 
for the newsletter AND to help out at the Entrepreneurs SIG meetings. The 
person submits the form and it is sent to the person who is the newsletter 
managing editor AND the person who is the leader of the Entrepreneurs SIG. 
(The email received by these two people can contain the same info but it 
would not go to leaders taking care of other tasks.)
