Guestbook

I have a need for a Guestbook for one of my sites but I can't find
anything pre-written that meets my criteria.

I wonder if anybody can recommend one that:-

- Allows multiple guestbooks to be run off the same script.

- Divides the entries into pages of 10 or 20 instead of merely writing to 
  a single, very long, page.

- Sends an e-mail to the person signing the book and also an e-mail       
   containing the details entered to the administrator of the book.

- Allows the submission form to contain more fields than the guestbook    
  would show, these extra fields would be contained in the acknowledgement 
  e-mail which the book would send to its administrator.

I can find plenty of scripts that fulfil most of these criteria but not
all. I'd be most grateful for a recommendation. 

Hopefully one day I would be able to write my own but my need is more
pressing than that.
