Hi Carolyn:
I use Acrobat 3.0 to create forms to put online.  You can see some of them
at http://www.mo-realty.com
I use .pdf files only when the form must look exactly a specific way, such
as reproducing government and business documents.  Using just html for such
things will give unpredictable results according to browsers.  But .pdf
forms are stable and I've never run across any trouble with it.
