Dreamweaver forms

Hello All-

I am wondering about my ability to put forms into layers in Dreamweaver.

Alright, I think I can place all my form data within a layer 
(although I am probably mistaken), including both starting and ending 
<form> tags.   When I put the form into the layer in Dreamweaver and 
proceed to create five objects for info gathering, the preview only 
shows two lines of text and objects, leaving off the last three 
objects.  Furthermore, it seems as though those three lines are 
excluded from the layer, and therefore not on the form because they 
show up in the upper left of the page.

The order of the fields should be of course, name, email, company, 
phone, comments.

If anybody can tell me what is happening or where I can find out what 
to change, I would greatly appreciate it.
