loads of questions there!

personally i have 2 approaches - during my day job I
do very little graphic design and mainly work from
comp screens provided by designers

then its a case of into photoshop, cut it up - maybe
use image ready depending on the page.

then into homesite, build tables and remove any
surplus images from image ready first.

then add javascript functionality that i know works in
the main browser the site is targeted at.

then its the first round of testing - if things work
fine in the primary browser then I'll add
cross-browser code to make things work in netscape for
example.

then test test and test some more

along the way is all the back end integration which is
specific to the cms/web server being used.

For my personal stuff i tend to work the same way
except I produce comp screens in photoshop as the
first step, then set up the directories on the ASP
server (usually IIS) then carry on as above.

tools: homesite, photoshop and image ready, and any
content creation stuff required - eg 3d
animation/modeling software or quicktime video
software.

As background, I've been in the industry around 5-6
years and tend to swing towards different areas from
time to time (eg I have done a lot of design work but
at the moment I'm mainly a developer)
