Use of ABBR and ACRONYM

When using the ABBR and ACRONYM attributes, would one use these *every* time
an abbreviation appears, or just for the first instance?  For example:

When writing <acronym title="Hypertext Markup Language">HTML</acronym>, the
author must be sure to properly use HTML tags.

or:

When writing <acronym title="Hypertext Markup Language">HTML</acronym>, the
author must be sure to properly use <acronym title="Hypertext Markup
Language">HTML</acronym> tags.
