ASP Problems

I have done everything that Microsoft said to do in article

To load information from a form into a database and an email.  On step
18 of the Default.htm they tell me to select the FILE UPLOAD tab - which
is not there.  So I skipped that step and went on.  

I went through every other step and uploaded the files to
http://www.risd.k12.nm.us/risd/nomination.htm and even tried
http://www.risd.k12.nm.us/risd/nomination.asp 

But the dreaded FRONTPAGE SAVE RESULTS COMPONENT shows up.  The server
extensions are loaded and working fine as you can see by
http://www.risd.k12.nm.us/bes/accreaderbytitle_author.asp

So can anyone tell me what I am doing wrong and how I can get the form
to work??
