"Mastering PHP 4.0" by Jeremy Allen/Charles Hornberger is an excellent book
for intermediate to advanced levels (my opinion).

For entry level to intermediate, the books by Julie Meloni ("PHP Fast & Easy
Web Development") are really excellent and easy to follow.

The Wrox books are really in depth but harder to follow and I personally
would put that in more of the advanced/PHP knowledgeable category.
