Strange Problem with Forms and MSIE 5

I've recently created an order form for a client and I'm experiencing a
peculiar problem when using MSIE 5.  This bug is rather strange, and
difficult to explain, so please bear with me for a moment...

The multi-page form works fine on several versions of Netscape, but when
using MSIE 5 the form does not seem to "submit" properly.  Approximately 50%
of the time when you click the submit button it sounds like the button has
been clicked twice - you can hear and audible "click...click" (two clicks),
and the instead of getting the next page of the form, you get a generic MSIE
page saying "unable to display page".  You then have to use your browser's
back button, backtrack, and resubmit the form for it to work properly.

Has anyone had something similar happen to them?  Any ideas on what may be
causing the problem?
