Ah - you assumed that I was including myself in the "big corporations". I
was referring to the majority of large software producers with online help
documentation. For example, MS documentation from Win2000 Server upwards.

I have developed an inhouse content management system and at present it
does use XML. The initial thought was to rely on XSLT to produce multiple
outputs. This is desirable whenever you have anything resembling a
print-based manual which also needs to be accessed via the web. At this
stage the XSLT hasn't eventuated and I'm only using one output format.

In Melb Uni's case - they applied a SGML solution to their undergraduate
handbook back in 1995 in order to produce web/CD/PDF/hard-cover
versions. Exactly the sort of thing you or I could accomplish nowadays
with XML (just easier :)
