Form query

Hi 'listers,

Have a request from a client that I would appreciate some advice on.

At present the form page on the site has the results of the form 
emailed to a single address using a form to mail CGI script provided 
by the hosting service. The client has asked me to provide a choice 
of four destinations (mailboxes) that the form results may be sent to 
by the user. The user should be able to select the destination for 
their enquiry using 'radio buttons' on the form i.e send to 
Department A, Department B, Department C or Department D.

Is this possible? Or do I need to use multiple forms to achieve this result?
