There are many free solutions (go to google do a search on 'survey =
script'.
If you wish to pay for a solution that works really well go to
http://www.cgiscript.net/  - they have a poll script that works like a
charm.  And you can set it up as a 'survey' also.  I am in NO WAY =
affiliated
with this software - just very happy with the results.
