CGI-SCRIPT Form

Hi all:
I am trying to use a simple cgi-script to send an e-mail to an address
(where I have an autoresponder) when a button is pressed.  The page is
http://www.cosmeticscolour.com/makeover.html and the buttons are the three
color buttons at the bottom of the form.  Everything is working except no
e-mail is being sent.  Could somebody please take a look at this for me???
