online counseling

Hi all,
I have a new customer that wants me to set up a site for her Counseling
practice.  She wants to be able to do online real-time counseling on a one
to one basis.  I know it could be done using ICQ or similar but that would
require all her clients to download something and install it before
proceeding.  That is not practical.  Any ideas on a good way to do this?  I
do not find anything quite like this when I do a search of the web.
