I have used Matt's Script Archives
(http://www.worldwidemart.com/scripts/) FormMail.pl for
this. Another way to get info via e-mail is to use
response-o-matic (http://www.response-o-matic.com).

Response-o-matic is fast, easy, and they write the code for
you, allowing you to dress it up to match your site or not
as you wish.
