Hi, guys & gals... oh, and you, too, Gio.

I've had a form on our company website for several years now.
It's at:

I have a customer that's used this form before, with no problem.
Today, he called and said when he clicked the "Submit" button,
he saw this on his Internet Explorer screen:

"FormMail
Copyright 1995 - 1997 Matt Wright
A free product of Matt's Script Archive, Inc."

The form is supposed to redirect people to a "Thank You" page.
This form came through to our company completely blank; it said:

"Below is the result of your feedback form.  It was submitted by
 () on Friday, December 3, 2004 at 10:58:18"

Several other companies used the form after this customer, and
they came through fine. I asked the customer having the problem
if they had made any changes lately, and he said that a while ago
they added a router. (He couldn't remember if he'd tried any form
submissions since the router had been added.) I had him try another
form I have for another company (it's in a pass word protected area),
and he got the same problem. The company the form gets sent to 
said they received the same blank form our company got, but that
it did have the correct subject line that I put into their code.

So, what do you people think.... is this a router configuration 
problem, or some change his ISP (bellsouth) recently made,
or something else I may be missing?

The people using our forms are industrial distributors, and most
of them aren't very computer literate, so I need to try to help
this person in layman's terms.

If anyone has any experience with this type of problem,
please help me!!
