Online Merchant Account

Hello all:
I have a new client who will be needing an online real-time merchant
account.  I have set up plenty of ssl order pages for clients who already
have merchant accounts and want to manually submit their online orders, but
this will be the first real-time account I have set up.  I have done a lot
of web surfing and have decided to use Authorize.net (AMS).  Anyone have
negative or positive experiences with them?  Also, they say what the total
cost for setup is and explain the monthly fees but I just want to make sure
there aren't any hidden costs I should know about.  Also, I'm open to
recommendations of other merchant account services.
