This is in response to the requests I received for the php code to use for
form submissions.  I have generated a basic form that you can use.

The form is located at http://www.webgraffix.com/phpform.phps

If you'd like to use the code, look at
http://www.webgraffix.com/phpform.phps and you will be able to see it.  Feel
free to copy it.  You will need to change the "mailto" value at the bottom
of the form and modify the fields of course, but I've included drop downs
etc so that it should contain most of what you may need. The only thing this
one doesn't do is validate the email address.  I will update it when I have
time and repost it.
