Try FormMail at Matt's script archive, at:
http://www.worldwidemart.com/scripts/

This is an internet classic, widely used, completely debugged, etc.
It has built in features like required fields and redirect.
Be sure to get the readme file and faq, too, available at the same url.
It works well enough that if something goes wrong,
I know it's me and not the script!
