One way this happens is accidental (:: blush ::), as follows:
Page A has a javascript which forwards you to Page B.
You visit A, and get java-forwarded to B. You hit your back button,
which takes you to A, which immediately sends you back to B,
around and around again.
Similarly,
Page A has a meta refresh tag with a time of 0 or .01 to page B.
You visit A, and get forwarded to B. You hit your back button,
it takes you to A, which immediately forwards you back to B,
around and around again.
