Hi All

I would like to ask 2 questions here... 

I have a web site that a company has looked at and wants to put up a banner=
 ad to advertise a new book that was just released. I was wondering 1) do=
 companies normally supply the ad (code) or do you (designer of the page)=
 create the ad? 2) what would be a reasonable charge per month for posting=
 a banner ad? (rough numbers would do fine)
