creating a Web needs survey

Hi - 

My husband & I have been charged with designing a
simple community site and would like to put together
some sort of member survey to determine what type of
content/features to add to the site.  

Can anybody please point me to some examples of this
sort of survey?  I'm on digest, so please cc me
