Need help with order form

Hi again,
Thanks to the multitude of responses I was able to get my order form to send without an error msg coming back, however, now the data I'm receiving is blank.  Below is a sample response.  Additionally, the clear form button doesn't work.  Can anyone help me figure this out? (PS, I plan on cutting the form into multiple pages, i know it's a bit long right now, but i want to get the important stuff working first).  Thank you.
