Replay Form (Guestbook) problem

We have been running a guest book for some time on our website (at 
http://www.assumptionabbey.com/guestbook.html )
and I have found that it only works with MSIE and that users on NN get an 
error message about permissions not being set correctly.  Our ISP assures 
us that the permissions are correct, and expect that they are since MSIE 
has no problems with it.

I suspect our coding (although this was a downloaded guestbook). Can 
someone take a look at the source code and see if there is anything that 
can be fixed to make this work correctly.
