We quote a flat rate for the entire project, based on a list of
deliverables.

We're considering restructuring our pricing for web design. Currently we
offer four levels of packages and a menu of individual items to tailor a
package to the exact needs. We have an hourly rate but only invoke that
for things such as custom artwork, consultation, and maintenance.
I know we can't discuss actual numbers on the list, but how do you
structure your pricing? Do you use a package-based system like we do or
are you strictly on an hourly basis? If hourly, I assume you try to come
up with a best-guess estimate, but certainly this must often run short
while the client makes a dozen changes, etc. How do you handle that?
