Hello smart people:

I am looking for information on how to design, download, or borrow a check-
list that can be given to the client prior to beginning web site development for 
them. They would fill out the form which would have items such as "font 
style=_______, colors____, purpose of this site____, profile of perspective 
visitors_____, etc. 

I have began to design too many web sites, only to have to go back and re-
do much of it because the client says, "I didn't request that!" I'd like a form 
that they would have to fill-out and initial prior to me doing anything on the 
design side of things.

Does such a form exist? I searched the archives under checklist but came 
up empty. If I make my own, I am afraid that I would over-look something. I 
was wondering if some experienced designers out there might have gone this 
path before and have something they've perfected they'd like to share--or 
suggestions of things to include into such a document.
