I recently implemented a policy which essentially forces a client to sign 
off on the website at each stage of the development.  I am thrilled to 
report that it WORKS!  Once a phase is completed the client reviews the 
site, approves it and faxes me a signed approval document which I provide 
before I continue with work.  The document alerts the client that further 
changes regarding this portion of the website will be billed at our hourly 
rate.  It has two benefits:

1)  The client seriously reviews that phase of the project.
2)  The client understands that he/she/they will be billed at an hourly 
rate for any other "fixes" or "changes" to that phase of the website.

By nature a website is a living thing.  It grows and changes (or it 
should).  It has been difficult in the past for me to shut-down that growth 
during the development process without discouraging the client.  The 
sign-off lets them know that other changes will need to be made, but in a 
later "phase" of the development.
