Critique please

Hello,
   I've setup a site that needs to be looked over.  It really is in the gray
area of commercial or not.  There is a registration page to access a free
health risk assessment, you are welcome to fill in your own information,
otherwise please use John Doe or Jane Doe and webmaster(at)healthstatus.com for
the e-mail.  I am looking for input on:
a) the whole look and feel of the site (duh?)
b) the ease of the registration process
c) the flow of the assessment questionnaire
d) if you should choose to print the assessment report out, how does it look
after printing.
   I'm new to the list but reviewed the archives and really did not see
anything like what I am requesting.  Apologies if I am breaking protocol,
and I ask for assistance in posting properly.
