Here's one that I got from "All the Sites"

 <form action="http://www.allthesites.com/cobrand/search/search.asp"> <DIV
class=menu-title>Search the Web</DIV> <input type="hidden" name="cbid"
value="58"> <select name="type">  <option value="all">All of the
words</option>  <option value="any">Any of the words</option>  <option
value="phrase">The exact phrase</option> </select> <input type="text"
name="query"> <input type="submit" value="Search"> </form>

If you use this code verbatim, I will get credit for your click-thru's so
you should sign up with them as an affliate to get your own click-thru
credits.
