Here's my $0.02.

Win98, MSIE4.x, High Color, 1024x768, 56K modem

HOME PAGE:
Nice gradient at the top.  Images load fairly quickly.  Could you make the
central picture a progressive JPEG?  I hate to see that big hole in the
middle stay open until the image is completely loaded.  Mouseovers work
nicely.  Slight horizontal scroll at 640x480.

PATIENT FORM:
See above.  Also, I like to put form fields in a table so all are left
aligned.  Fax number is not very legible at 640x480 (hiding in lower
gradient)

GENERAL:
Extremely professional.  Great look, and absolutely consistent throughout
the site.  Good use of graphics.  Good load times for us modem users.  Super
job.
