Hi,
I do it by using the Edit>Define Pattern option in Photoshop.
First grab this Zip file from my server:
http://961.dns2go.com/downloads/patterns.zip
You can see the examples of the images I created at:

If you are prompted for a password:
UserName: guest
PassWord: 961guest
Ok... Open the Zip file and export the images.
Open each .PSD in Photoshop
For each image in the Zip file do the following:
1: Click on Edit>Define Pattern
2: Click ok
3: Close the image
To apply the effects:
1: Open the image of your choice
2: Create a new layer in the image and make sure it is selected on the
layers pallet
3: Click on Edit>Fill
4: Under the "Use" Menu select Pattern
5: Under the "Custom Pattern" menu select the pattern you wish to use
6: Click Ok
Scanline tips:
For different effects you can change the "Blending Mode" options on the
layers pallet. I generally like to use Overlay or Soft Light for a more
subtle effect. Using Saturation also gives a nice effect.
You can also get this effect using Xenofex. You will have to apply it to
the actual Image layer itself, rather that a new layer so it's not as
flexible.
Choose the Television filter from the list and set the following:
Scanline Strength = 100
Scanline Thickness (pixels) = 1
Every other setting should be set to 0
Dot tips:
To make this come off correctly you need to make sure your image and/or
canvas size ends with an odd number. If you don't do this you will end
up with a 1 pxl solid line on the right and bottom sides of the image.
201 X 201 pixels = correct
200 X 200 pixels = incorrect
I hope I covered all the steps and I hope this helps a bit.
