I always make them manually, and though I, too, am tempted to bail out to a
vector program, Usually, I'll do them in Photoshop.
The key elements are an image the size of the finished rectangle you want,
the creation of a black "doughnut" on a white background with the desired
diameter (i.e., marquee select a circle with desired diameter--always
divisible by two--and fill with black; marquee select another circle with
diameter smaller than first by twice the desired thickness of the line,
and, using the Ctrl+Alt [Command+Option on the Mac?] and the arrow keys,
move the selection outline to the exact center of the black circle, and
fill with white). Cut the doughnut into quarters and move each of the
quarters out to the corners of the image. Then connect the arcs at the
corners with black rectangles, select all, invert the colors and cut to the
clipboard for pasting into your other image as an alpha channel.
