This may be too basic or just plain off, but I had a similar problem and
found that the resolution of the images were different than what I had been
using.  Instead of the 72 pixels per inch, I got hold of a 300 ppi image.
It drove me nuts until I noticed the difference in resolution.  Once I
synched everything up, I had no further problems.  PS did remember the
higher resolution setting for a long time as well.
