previously working js script not working anymore

Hello Web Gurus,
I have a page within my site that used to work, & other pages that use the
same script for the same effect are working, but this page isn't.

When one clicks on a choice in the lower drop-down list, some text is
supposed to appear, as it does on other pages.  This is probably simple, but
I don't understand why something that used to work is no longer working.
