Form processor on an nt box, using activestate perl?!

Hi.

I'm having configuration troubles with a free form processor I'm using
for the first time, and, since I know naff all about perl (or, come to
that, nt!), figured this was the place to cry "AAGH!".

First things first: This is for a website, commercial, the first one
I've done involving cgi/perl. The server is an nt box, using
activestate's nt version of perl5. The space is on fasthosts.com
servers. The host reseller (a freind of mine) is on holiday, and I
can't get to him (getting a picture of fubarbundy yet? I may, by the
end of this, be bald as a coot!).

The form processor comes from FreeScripts.com:

And here's the all important error message:
    CGI Error
    The specified CGI application misbehaved by not returning
    a complete set of HTTP headers. The headers it did return are:

    Can't locate cgi-lib.pl in @INC (@INC contains: c:/Perl/lib
    c:/Perl/site/lib .) at e:\domains\moorcroftcollectoronline.com
    \user\htdocs\cgi-bin\form_processor.pl line 19.

As far as I can see, I've configured what needs to be configured, but,
given the above, I've fouled up somewhere - anyone got any hints,
idea, or better yet, tell me exactly where I screwed up, please?!

Please note that I'm on the digest version, so replies may be slow in
