Here's what I use and have no complaints with:

<?xml version="1.0" encoding="UTF-8"?>
<!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN"
"http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd">
<html xmlns="http://www.w3.org/1999/xhtml" xml:lang="en" lang="en">

I don't use the meta tag at all.  I probably got this heading from an
iwa-hwg instructor from one of the classes I have taken.

Also, just to confirm the validator being used, I use:
http://validator.w3.org/

Am trying to go the xhtml way - but although using doctype my xhtml book
advises, but comments from w3c validator makes me wonder whether my
