Sending Forms Through Email

Sorry If you have gotten multiple posts of this email, because I think I
forgot to send in Plain Text.

Hello List. I am sending a form consisting of a hidden field and a submit
button through email using the CDONTS.NewMail object. However, when I try
and send it, I can't get the right apperance. When I don't put any
attributes in the form tag and just put <.form> the button appears fine, but
doesn't go to the right address. Here is my code (I haven't put the hidden
field in yet):

objMail.To = "matthew(at)ohlman.com"
objMail.From = "ohlman(at)ohlman.com"
objMail.Subject = "You Have Recieved An EMAIL!"

objMail.Body = htmlText
objMail.send

The email sends fine, but it looks like this:

I have tried using something like
htmlText=htmlText & "<form action =" & "test.asp" & ">"
but it didn't help either.
