i don't think it is possible with any of the "standard" coding methods
(html, xml, asp, css, etc.)
here are a couple of suggestions:
1. the new Macromedia Flash 4 allows for form creations that has the look of
a "printed" form.
2. create an Adobe PDF web form. i would recommend this, especially if you
desire the look of a printable form!
