mouse pointer change?

I'm curious and I figured I'd check to see what you think.  The other day 
someone told me it's possible to change the mouse cursor to an image of 
your choosing when you move the mouse pointer over a link.  They also said 
it's possible to create a hidden link that only shows up when the CTRL key 
is held down.  I didn't believe them, but they swore it's true.  Have you 
heard of anything like this?  Or for that matter, any idea on how you'd 
implement something like this?
