Frontpage mail form

I have created a web site with a form that I would like to email at
http://www.risd.k12.nm.us/partners/partnersappl.htm .  The Frontpage
extensions are loaded and I have configured the SMTP server information,
but cannot get the email form to work.  I have followed all instructions
and am at my wits end.  Can anyone please tell me why the page says
[Frontpage Results Component] and why the form does not work?
